Creating Forms
The signup form is the form that clients fill out when they first join you. It collects all of the information you need in order to individualize the client's nutrition targets and habits.
Apotheo comes with three pre-built signup forms:
Basic Only - contact details, starting photos and measurements. Use this form when adding clients that have already provided details through your previous onboarding system.
Nutrition - Basic Only + detailed health, lifestyle, and nutrition history.
Nutrition & Training - Nutrition + training history and equipment availablity.
Signup forms can be customized under Settings > Onboarding > Signup Forms
To customize your signup forms, we recommend creating a copy of one of the existing forms, then editing the copy.
Changes made to signup forms will only apply to clients added after the changes were made.
Assign the signup form to a service under the Service settings at Settings > Service Settings > Services.
You can override the default form when adding a client. For example, if you're adding an existing client during your trial you might want to choose the Basic Only form so that they don't have to fill out the information they've already supplied via your previous onboarding system.
