Creating application forms

Application Forms



If you receive a lot of coaching applications, the application forms feature helps you keep track of who applied, how you responded, and whether the application was submitted by a returning client.

Apotheo offers two different workflows for application forms:

Review / Accept - clients are not added to Apotheo until you've reviewed their application form and accepted them.
Take Payment - clients set up payment directly from the application form and their account is created automatically.




Creating a form



Go to Settings > Onboarding > Application Forms and click Add form.

All applications forms contain the following fields by default:

Taking payment



If you want clients to be able to sign up and pay directly from an application form, you can enable the Take Payment option and select a pricing option.

When the form is submitted, the client will receive an email with their account details and be forwarded to Stripe Checkout to set up payment.

In the Review / Accept workflow clients are prompted to set up payment the first time they log in after you create their account.

Setting up your thank-you page



After a potential client submits an application they'll be redirected to a thank-you page. You can use the default thank-you page, or send them to a custom URL on your website.

Customizing the form appearance



If you want to customize the styling of the form, you can add your own CSS code to the Custom CSS field. This step is optional.
