Configuring Check-In Questions


Check-in questions are the questions your clients answer each time they submit a check-in. You can create multiple question forms and assign different ones to different services.


Viewing Your Question Forms


Navigate to Settings > Check-In Questions to see a list of all your check-in question forms.

Each form shows its name and action buttons to preview, edit, or delete.


Creating a New Question Form


  1. Navigate to Settings > Check-In Questions
  2. Click Add form
  3. Use the form builder to add and configure questions
  4. Save the form


Question Types


The form builder supports the following field types:


  • Short Text — single-line text input. Options: max length (1–255), min length, placeholder, required, hidden.
  • Long Text — multi-line text area. Options: max length (default 1000), min length, placeholder.
  • Number — numeric input for whole numbers.
  • Float — numeric input that accepts decimals.
  • Scale — a 1–10 rating slider.
  • Yes/No — boolean toggle.
  • Choice — radio buttons for selecting one option from a list.
  • Multiple Choice — checkboxes for selecting multiple options from a list.
  • Dropdown — a dropdown menu for selecting one option.
  • Date — a date picker.
  • File Upload — file attachment with optional file type filters (image, video, document, audio).
  • Content — display-only text or instructions shown to the client (not an input field).


Field Options


Each field supports the following options depending on its type:


  • Required — the client must answer this question before submitting
  • Hidden — the field is hidden from the client (useful for internal tracking)
  • Help Text — guidance text displayed below the field
  • Placeholder — placeholder text shown inside the input
  • Min Length / Max Length — character limits for text fields


Editing a Question Form


  1. Find the form in the Check-In Questions list
  2. Click the Edit button
  3. Modify your questions and save

At the bottom of the form editor, two buttons let you add content:


  • Add Fields — adds a new question input (short text, scale, file upload, etc.)
  • Add Context Text — adds a display-only content block for instructions or explanatory text between questions. Context text is shown to the client but doesn't collect any input.

Deleting a Question Form


Click the Delete button next to the form. A confirmation dialog will appear before the form is removed.


Note: Deleting a question form won't affect past check-ins that used it, but clients assigned to a service using this form will need a replacement.


Previewing a Question Form


Click the View button next to any form to see a preview of how it will appear to clients.


Assigning to a Service


Question forms are assigned to services, not directly to clients. To use a question form:


  1. Go to Settings > Services
  2. Edit the service
  3. Select your question form in the Check-In Questions dropdown


All clients on that service will then use this question form for their check-ins.
