Application Forms

Application Forms


Application forms let prospective clients apply to work with you before you accept them. Unlike sign-up forms (which are completed by clients you've already added), application forms are public-facing — you embed them on your website or share a direct link.


Creating an Application Form


  1. Navigate to Settings > Application Forms
  2. Click Add Form
  3. Configure the form and save


Application form editor


Form Fields


  • Name — the name of this application form (internal reference)
  • Application Form URL (Slug) — the URL slug for the form. This determines the direct link.
  • Confirmation Page URL — where applicants are redirected after submitting


Coach Assignment Options


  • Available Coaches — select which coaches can be assigned applicants from this form
  • Allow applicants to choose their preferred coach — let applicants indicate which coach they'd like to work with
  • Allow applicants to select "No preference" — adds a "No preference" option to the coach selection
  • Allow second preferred coach selection — let applicants pick a backup coach (useful with waitlists)
  • Assign to coach with fewest clients — automatically assign applicants to the least-loaded coach
  • Assign in round-robin fashion — distribute applicants evenly across available coaches


Self-Signup Options


  • Take payment — enable self-signup with payment. When enabled, additional fields appear:
  • Service/Client Type — which service to assign
  • Pricing Option — which pricing to use


Custom Fields


Add custom questions using the field editor (same as sign-up forms). Every form includes First Name, Last Name, and Email by default.


Sharing Your Application Form


After creating a form, click its name in the list to view sharing options:



A shareable URL you can send to prospective clients or post on social media.


Embed Code


An iframe embed code you can paste into your website. The preview shows how the form will look embedded on a page.


Reviewing Applications


Applications List


Click Applications in the sidebar to view all submissions. The list has tabs for:


  • All — every application
  • Unread — new applications you haven't viewed
  • Pending — applications awaiting your decision
  • Waitlist — applicants you've placed on hold


Your dashboard also shows application counts that link to filtered views.


Application Details


Click an application to see the full submission, including all form responses and timestamps.


Taking Action


From the application detail page, you can:


  • Reply — send a message to the applicant
  • Accept — approve the application and generate a login for the client
  • Reject — decline the application
  • Waitlist — place the applicant on hold for later review
  • Archive — move the application out of active view
  • Restore — bring an archived application back


When you accept an application and generate a login, the applicant receives an email with a login link and is added as a client.


Editing an Application Form


  1. Find the form in the Application Forms list
  2. Click the Edit button
  3. Update the fields and save


Deleting an Application Form


Click the Delete button next to the form and confirm. Past submissions are preserved.
